District Meeting Report (Form 2)


If your Healthcare Group cannot attend your District's meeting to present the President's Report, please complete FORM 2 in the packet you received and mail it to the District Coordinator, or complete the online form below.  This form needs to be received by TAHV at least two weeks prior to your scheduled district meeting.  The District Meeting Schedule is posted on this website.

After presenting your oral report at the meeting, be sure to leave a copy with the District Meeting Coordinator or TAHV First Vice President. 

This report is important to TAHV in gathering statistical information.
